Patient Portal

Welcome to the Patient Portal online Services for Clearwater Family Practice.

Beginning September 1st, 2014, federal guidelines mandate that physicians provide selective access to medical records in an easy, safe and secure format, for patients to become more interactive with their healthcare. The Patient Portal is designed to allow patients to register and establish a patient account to review some of their medical records. The following explains how this is accomplished, and what patients are required to do in order to access select medical records.

Clearwater Family Practice is constantly positioning itself as the Provider of Choice in Family Practice, and has established this Patient Portal as a means of providing choice for its valued patients and family.

Instructions for use:

The Patient Portal allows you, the patient, to communicate with your doctors, nurse practitioners, and medical support staff in various areas. Follow the instructions below to begin.

  1. Register: Each patient must register on the patient portal and create an account in order to access information through the Patient Portal. To register, Clearwater Family Practice will send you a username and your initial password, which you will be required to change. Your password MUST have FOUR characters and can be either letters or numbers.
  2. Establishing an Account: An account is established when the patient enters the username and password provided in the email sent by Clearwater Family Practice.
  3. Login: By entering the provided username and password, you are able to login to your account. You will then be able to access selected medical records.

The Patient Portal will also allow you to:

  1. Complete paperwork and change information such as address, provider and provider number.
  2. View selected laboratory results, if requested.
  3. Communicate with medical staff.
  4.  View your care plan.
  5. Submit requests for appointments, prescription refills and more.
  6. Receive educational materials.

If you have forgotten your password, there is an easy process to establish a new one.
